2023 FUHSA Physical Screening Exercise: All You Need to Know

The Federal University of Health Sciences, Azare (FUHSA), extends an official invitation to all candidates who have applied through UTME and Direct Entry for the 2023/2024 academic session. The university has organised a Physical Screening Exercise that will occur on specific dates for different programmes.

Detailed Schedule of Physical Screening for Various Programmes
Medical Programmes: Medicine & Surgery and Dentistry

For candidates who have applied for the Medicine & Surgery (MBBS) and Dentistry (BDS) programmes, the Physical Screening Exercise will be held on Wednesday, 30th August, 2023.
Nursing Programmes

Applicants interested in the field of Nursing should mark their calendars for Thursday, 31st August, 2023.
Radiography and Nutrition & Dietetic Programmes

Candidates applying for Radiography or Nutrition & Dietetic programmes will have their Physical Screening on Friday, 1st September, 2023.
Timing and Venue

The screening activities for all programmes will take place daily from 9:00 a.m. to 4:00 p.m. The designated venue for these crucial screenings is the University ICT Centre in Azare.
Document Requirements for Physical Screening
Compulsory Documents

Candidates are required to bring the originals and one set of photocopies of the following documents:Acknowledgement Form, printed from the FUHSA Post-UTME Portal
UTME Result Slip
O-Level Result (either WAEC or NECO)
A-Level Result or First Degree certificate (applicable only for Direct Entry candidates)
Scratch cards for verification of ‘O’ Level result(s) submitted
Evidence verifying Local Government Area of Origin
Birth Certificate or a Sworn Affidavit of Age Declaration
