Collect and fill out forms at the College of Nursing Department.
Submit filled forms immediately after completion at the College of Nursing Science Department for verification.
Photocopy the Form, Receipt of payment, and other Documents.
Return the photocopied Form, Receipt of payment, and other Documents to the College of Nursing Science Department.
